The Chatham Council on Aging Outreach Coordinators work with seniors in the Chatham community and their families to quickly identify issues and correctly refer their clients to the appropriate helping organizations. Our Outreach team embraces the ever-increasing complexity of outreach needs through ongoing training in order to meet the needs of the community. Identifying the elder care issues and learning how to address these issues helps us provide sophisticated, specialized services including caregiver support and education, long term care planning, crisis management, aging services information, and benefits assistance to better serve community members.
Transportation for Chatham Residents The Chatham COA can provide transportation on our wheelchair-friendly vans Monday through Friday for local errands and grocery shopping. Reservations must be made 48 hours in advance by calling 508-945-5190.
COA Café Gourmet Lunch Chef Heather Bailey joins us each Monday (except holidays) to prepare a fresh gourmet lunch for our hungry guests. For only $7, you’ll get a bread basket, entrée, dessert, and beverage. Meals are also available to go! Simply call the Senior Center at 508-945-5190 by the Friday afternoon prior to make your reservation or order your to go meal. Menus are listed monthly in the FLASH; this event is open to anyone of any age!
COA Cafe Gourmet Soup-To-Go Chef Heather Bailey makes delicious, gourmet soups weekly- available while supplies last! Only $4/pint, you can freeze them or enjoy them right away. Congregate Luncheon Lunch is provided by Elder Services of Cape Cod and the Islands on Tuesdays and Thursdays at 11:30am. Reservations must be made one day in advance. Call the Nutrition Site at 508-945-1783 to sign up. Transportation can be provided by calling 508-945-5190. Donations are encouraged.
Colin Campbell Computer School Run by the Friends of the Chatham Council on Aging, this computer school is open to anyone (not just Chatham residents!) aged 18 or older. Topics covered include iPhone, iPad, Mac, Windows 8, Microsoft Office programs, and many more. Space is limited, call 508-945-5190 to register.
SHINE Counselor (Serving Health Information Needs of Elders) Free health insurance counseling is available at the Senior Center on Tuesday mornings. Call 508-945-5190 for an appointment.
Weekly Blood Pressure Clinic Have your blood pressure checked by the staff of the Visiting Nurse Association of Cape Cod at the Senior Center on Wednesday mornings from 9-11am. Call 508-945-5190 for an appointment.
Foot Care Nurse Appointments available monthly, by appointment only, $30. Call 508-945-5190 to schedule an appointment. Home visits also available for $50 each.
